To paraphrase Theodore Dalrymple, “distress” always expands to meet the supply of publicly funded services available to reduce it. So the idea that Medicaid expansion would expand Medicaid was never in doubt. If you build it they will come. If you build it with other people’s money, and back it forever, they may come in excess, and that is what has happened in New Hampshire.
